2009 SKEA Achievement Award

Eligibility
1) Applicant must be a senior at a public high school, equivalency program, or have graduated within two years prior to application from said program.
2) Applicant's parent or guardian must be an active SKEA member or applicant's deceased parent or guardian must have been an active SKEA member at the time of his/her death.
3) An individual may apply one time only.
Selection
The Salem Keizer Education Association will appoint three (3) retired public school teachers to serve on the SKEA Achievement Award Selection Committee. The following criteria shall be given equal weight in selection:
Grade point average
School and community activities
Performance in required high school classes
Completion of application form
Receipt of Award
Award recipient must show evidence of high school diploma or program completion. Additionally, recipient must enroll in a vocational program, community college, or four-year college/university within one calendar year of award selection. Failure to do so will result in forfeiture.
Amount
2-$1,000 non-renewable awards and 4-$500 non-renewable awards
At least one vocational program/community college award will be given, if
there are applicants.
Application Procedure
Applicant must:
1) Complete the attached application form, adding attachments only if absolutely necessary.
NO FACSIMILES, DUPLICATIONS, SCANNED COPIES WILL BE ACCEPTED.
2) Enclose a copy of official high school transcript (whether in high school or college) and college transcript if applicable.
